I agree that Noémi Kovàcs may carry out an operation on my body during the procedure. I was informed that a tattoo constitutes an invasion of the integrity of my body and therefore an intentional bodily harm. This consent eliminates the illegality of the corresponding articles in the Swiss Criminal Code. By registering, I give my consent to the planned procedure, including the necessary pre- and post-treatment. By registering, I also confirm the accuracy of the health-specific questions I have provided. If information is withheld that poses a health or even life-threatening threat to the customer or studio staff, legal action may be taken. With my signature, I release the studio and Noémi Kovàcs from all responsibility and release the studio and Noémi Kovàcs from all legal claims. Liability and compensation claims, including for complications of any kind and consequential damage, are therefore completely excluded.
